MHD Services Repair procedure
After submitting this request you will receive an email with your repair job ID and login details. Please print this email, pack with your phone and sent to the below address:
To check your mobile phone IMEI please enter the following sequence in your mobile phone *#06#. IMEI is a 15 digit serial number. Alternatively when you remove the battery, under the battery you can find a sticker with a barcode and the15 digit IMEI.
In the “Comments” field please write any special delivery instructions or requests you may have.
Once we receive this form and the faulty unit will contact you within 24 hours of receipt of the faulty mobile phone.

Pickups and Drop off: pickups and drop off are between the hours of 10:00 and 14:00, Monday to Friday. For other hours, please contact us to make and appointment.
Repair Terms and Conditions
1. By submitting the repair authorisation request the Customer agrees to the following terms and conditions 2. The Customer agrees that information supplied to MHD Services is accurate 3. MHD Services reserves the right to refuse or charge providing service in the following cases a. Previously repairs or modifications have been undertaken that were unauthorised b.IMEI/ESN or other serial number or MHD Services warranty label is damaged, missing or does not match the handset software c. Damage is caused by abuse, misuse, liquid ingression and improper ventilation 4. When a repair is deemed to be outside the manufacturer's warranty the Customer will be contacted and provided with a formal quotation before any repairs are undertaken. MHD Services will not commence repairs when such a formal quotation is provided until the Customer authorises MHD Services to repair the device. 5. A quotation fee of $35 ex GST will be charged for repairs that are deemed to be outside the manufacturer's warranty. If the Customer accepts the quotation and proceeds with the repair, the quotation fee will be refunded. 6. A handling fee of $35 ex GST is charged on all warranty repairs. 7. The shipping charges to MHD Services's repair centre and from MHD Services's repair centre are the responsibility of the Customer. 8. These terms and conditions cover the contract between MHD Services and the Customer and supersede any representations, warranties or assurances made by MHD Services at any time. 9. The Customer acknowledges that important data and configuration settings (PDAs, Blackberry devices) may be lost while the device is being repaired. MHD Services accepts no liability for any loss of configuration settings and/or data including but not limited to ring tones, logos, downloads, music, phonebook, pictures, photos, files, Blackberry device settings and any other information, documents and/or files. It is the responsibility of the Customer to backup the handset before submission of repair. Customers are advised to keep a hard copy of any information stored within the device and/or regularly backup their device. 10. The Customer agrees that IMEI/ESN and/or other serial numbers may change. 11. A three (3) month warranty on parts and labour of repairs done by MHD Services applies to all completed and paid for repairs. If the Customer has outstanding accounts with MHD Services the Customer acknowledges that this warranty may no longer apply. 12. Some liquid and physical damaged mobile phones may take longer than 48 hours to repair
